How do I register my Connecticut Foreign LLC?

1. Make sure your LLC’s name is available.

  1. Name and address of the person filing—this is where the state will send your confirmation.
  2. The legal name of your LLC.
  3. DBA (or alternate) name, if applicable.
  4. State where you initially formed your LLC.
  5. Date your LLC was initially formed.
  6. Date your LLC began or will begin conducting business in Connecticut.
  7. Principal office address (physical only)
  8. Principal mailing address (street or P.O. box)
  9. Business address in your home state.
  10. Company email address. You may select “none” if you don’t have one.
  11. NAIC code. To find your code, check the NAIC website.
  12. Name and street address of your Connecticut registered agent.
  13. Your registered agent’s signature.
  14. Member/manager information. You must include the name and address of at least one member/manager.
  15. Name and signature of the person filing.
